Abstract: Torsiversion of a tooth is defined as its rotation along its long axis. There are very few published reports on torsiversion of mandibular third molars. Neiburger1 reported an incidence of 42% in a group of Mid–Western American Indians. He also reported a 2% incidence in a mixed population of Caucasians, Negros and Latins in America.

Most reported cases observed a 30o buccal rotation. We report here a case of 90o buccal rotation of a right mandibular third molar with an incidental finding of a paradental cyst attached to its ‘distal’ aspect.
